I understand that I must be at least 18 years of age to consent to services with Sara Lapides. As a client of Sara Lapides participating in e-mail or telephone counseling, I submit that I am not suicidal and have no thoughts of harming myself or anyone else. I also understand that Sara Lapides, as a licensed Marriage and Family Therapist in the states of North Carolina and California, is required by law to break confidentiality and make the appropriate legal notifications if she has reason to believe that I am involved in child abuse or neglect, or if I intend to harm myself or someone else.

Sara Lapides is bound by the legal mandates for Marriage and Family Therapists in the states of North Carolina and California. Counseling services are offered in person, by telephone or e-mail only in the states of North Carolina and California. Coaching services are offered nationwide.

I understand that there is no guarantee as to the outcome of counseling provided by any mental health professional, and that I may terminate counseling at any time, for any reason, and without prior notification or explanation to Sara Lapides. I also understand that I am responsible for the privacy of telephone & e-mail sessions on my phone line and computer, and that Sara Lapides ensures the privacy of all e-mail exchanges and telephone consultations between Sara Lapides (provider) and me (client).

I have been informed of the fees for services as per the Fees & Payment section of this Website. I agree to pay for all counseling or coaching sessions with Sara Lapides. I understand that there may be times when a session has to be cancelled, and that if Sara Lapides must cancel a session, she will reschedule our session for another mutually-agreed upon time.
